The FTC provides an ID Theft Affidavit to help victims of identity theft quickly and accurately dispute new unauthorized accounts. It is especially helpful in cases where consumers are unable to file or obtain a police report. Some creditors will accept this affidavit instead of a police report. Also, when you contact creditors to dispute charges based on the fact that you were a victim of identity theft, they will likely require you to complete their own affidavit form, but might accept the FTC's version instead. Using only one affidavit form will save you time. Be sure to fill out the affidavit completely and accurately.
The FTC ID Theft Affidavit can be downloaded below. Remember to use certified mail, return receipt requested, so you can prove your documents were received. Always send copies to creditors and keep originals for your records.
